    Worker dies at Exxon Mobil Singapore refinery

    Exxon Mobil Corp. says that one worker was killed and another critically injured late Monday during maintenance work at its Singapore Jurong refinery.

    Exxon Mobil said in a statement Tuesday that two contractors were found unconscious in an enclosed space filled with nitrogen. A 34-year-old Indian national died later at a hospital while the other worker is in critical condition.

    The company said it is cooperating with authorities to investigate the cause of the incident.

    Exxon Mobil said it suspended maintenance at the 605,000 barrels per day capacity plant, which manufactures a wide range of fuels, lubricants and specialty products that are sold in Singapore and exported to Asia.

      They were creating and developed fantastic rules and regulation for contractors but they were not yet being followed their procedure completely. The worse things is copying of thousands of documents like Risk Assessments and other safety procedures. What is the meaning of SPSA and so on? How did they apply with AAA. First, are they Access any jobs details or not? PLEASE be sure with your IN HOUSE RULES through to your staff in heart. Please do care all of LIFE in your workforce.

      This incident is happend due to lack of supervise of Exxon mobil staff. The only word we come to know when we do safety training is Exxon mobil is very tuf on safety of the contractors. still there are plenty people who dont know how to behave in exxon mobil. they dont provide anything. and no surprises. every thing depends only on the contractors who is involved in the job to provide everything. contractors think money and not the safety of the people. THERE ARE SO MANY COMPANIES THAT BLAME SAFETY FOR THE SLOW PROGRESS OF THEIR JOB. IN THE MEETING THEY SAY IF ANYTHING UNSAFE "STOP THE JOB" BUT WHEN WE STOP THEY SACK US OR ELSE THEY CUT DOWN OUR OVERTIME, IF NOT NO BONUS. DIFFERENT PROJECT MANAGER HAVE DIFFERENT KNOWLEDGE. SAFETY SHOULD NOT STOP THEIR JOB, LET THE PROGRESS HAPPEN, IF ANYTHING HAPPEN ULTIMATELY BLAME SAFETY.

      EM staff not working at site. They go to site for complete their target to write observation only, and they write some funny thing that don't hurt people. But they fail to find what can kill people.EM management need to weak up.

      i've been working in Oiltanking for maintenance of Propelyne tank (Shut down) and connection of new Naptha pipe to the existing line. Well, the safety there are strict and daily after gas checking, i am the one going into the tank to reconfirmed and giving the green light with the HSSE of Oiltanking. all was done well.
      as i knew, while working in SPT, the Exxon HSSE are more strict as compared to Oiltanking. the confine space oxygen level are more control than the MOM specification. in fact, three or four safety will check in the morning and after lunch.
      in conclusion, the HSSE or safety with dedicated safety responsibility is a must when doing any chemical or confine space task.
      as FT, they are the safety and they are the site supervisor. to safe cost. my understanding, safety personnel is an individual section abd cannot perform two task at a time. believe me, most local companies try not to hire local safety as their wages are high, but their responsibilities also high.

      guys,pls take note all PTW on ever works come with permit as issue by HSE department
      HSE personal reponsible to check and inspect b 4 sign and approve to works permit
      they are well train with all certificate but sad to say not with their heart by only monie

      why is not foreman and safety officer around at point of time? why is workers allowed to go in to confine space when rescue team is not around? from what i know worker should not go in to confine space when rescue team is not around. is permit and safety check done? what is em safety officer doing???????????

      why is everybody jumping into conclusion when the investigation report is not out?
      it might be the assessor's negligence,
      or the workers',
      or the SO's, etc etc etc.
      Why not just wait till the report is out?

      Hey Bravoo, pls don't just jump into conclusion. There are experts who will rectify what went wrong. Yes this is human error, but shouldn't u allow them time to find who's responsible for this accident.Pls don't just say NO safety No work....does not make sense..This is a very big company , not a kopitiam

      It is interesting to read all the comments written down by the so called experts. I agree with the lot that say lets wait for the investigation report before we make any comments about anyone.
      Its unfortunate that there was a fatality and prayers for all affected by it. RIP

      Someone who is responsible for his death should be penalised heavily for not ensuring the enviroment is safe to proceed, taking a worker life for cheap.
      Where is the exhaust blower for confined space? no readings taken?

      It's difficult to coment at this moment until we have full details of the incident. In most accidents, there are many contributing factors or causes (mainly human factors or rush work) that lead to an accident or mishap. So we should not make assumptions and simply blame someone or the company.
      I wish to suggest that workers especially in the heavy or petrol chemical industries should have some years of experience or knowledge in the trade they are working. This will certainly helps in preventing accidents in addition to undergoing necessary relevant training which is mandatory by law.
      Hope the relevant authorities will take a positive approach in making our industries safe as training alone will not help to improve safety in the industries as knowledge, skills and attitudes are important elements in avoiding risks at the workplace.

      well, sometimes it is fate. but mostly FT only do as were told. my experience, FT without ungoing the WPLN but having foreign cert are allowed to take the Advance cert in WSH. YES, IT IS HAPPENING! we, Singaporean have to be checked all educational level. so for this case, is the safety personnel a FT or Singaporean. i'm not looking down on them, but the sense of responsibility is totally different.

      i think the authourithy need to be more strict in issue the cert. my suggest, the coordinator need to take a point system too as the WSH officer. most local companies just have the thinking of having BCSS, they are qualified to be safety personnel. ONLY THOSE WITH WSH Level A, B & C are allowed to carried out the task.

      Just another lapse of safety. Safety checklists must be in placed before one start to carry out the works. Safety officer must be present on site, to brief on the work method before any commencement of works. Submit the SMS ( Safety management system ) prior carry out the works.

      enforcement and double checking is very important before entry to confined spaces even worker hold valid entry permit,no point pointing finger. life loss very sad, GOD BLESSED.

      I'm so sad to hear this. Poor guy! but.....
      Afterall, safety is not just the company issues, one should be resposible for his/her own. I guess they just need to tighten up and have refresh course( keep it short and sweet, i have been too many and it is simply just boring) those foreign workers as they tends to neglect and "take for granted" attitude. Of course, not matter how safe a place is, accident still will happen.

      Just like how some pedestrian being knocked down, as they simply do no use or understand or obey traffic rules. what can i say?

      Firstly the safety department and the company concerned should be held responsible for the neglegience
      1) where is the clearance of confined space documents
      2) who had cleared the confined space
      3)where are the risk assessment for all the works carry out
      4)where are the safety watchers
      5)Where is the safety officer/safety supervisior located
      6)who had issued the permit to work
      7)check the number of supervisior against the number of workers against the number of locations they workers were working against the number of supervising engineers and representative of the refinery.(this will clearly show there will not be sufficent managemnet staff to supervise)

      Please note there are many policies in place but actually in the field of work how many is implemented.when an accident there will be all this documents surfacing but is it implemented is a question.
      there fore beforecommencement of any works there should be an acknowledgment of this to MOM so that if any incidents happen then MOM together with insurance can check wheter there is any neglegience.
      Secondly MOM safety dept should have an office at Jurong island to carry out random or spot checks at any time round the clock at any of these plants.
      There is always failure in the system.No contractors wants have an extra men or safety to be implemented at their cost which eats into their maximum profit.
      All the accidents and incidents are taking place due to shortfall of qualified supervisiors who are safety concern.If the safety supervisior or the safety officer acts strictly with the rules he is unemployed or loses his job.
      PLEASE LOOK INTO ALL THESE VERY SERIOUSLY before we lose another life or lost time of injury.
      All companies have to put a safety deposit or bankers gurantee before they engage in this jobs.This should deter any companies of losing their deposit if there is an accident or incident.

      There seems to be many ppl pointing the finger on safety personnel. Actual fact is when accident happen not only the injured is at fault or the safety personnel is at fault but somewhere down the line the system has been breached by human itself. So stop pointing finger at any individual and get your facts right before you comment anything. Inert entry is a high risk job and has to go through the safe operation committee approval before the work can be carried out and mind you no EM staff in the right mind will leave it to the contractors to perform this job without any proper supervision and proper procedure in place. Since the last accident which happen in 2007, they have step up their safety enforcement and I believe nobody wants accident to happen. Therefore let’s wait for the outcome of the investigation so that we can learn from this mistakes.
