Human Error Cause of Kunduz Hospital Strike, US Military Says

The top US commander in Afghanistan, Gen John Campbell on Wednesday, November 25, shared the results of investigations into a US airstrike on a Doctors Without Borders hospital in Kunduz in October. The strike killed 30 people. The commander said that the strike was the “direct result of human error, compounded by systems and procedural failures.” The commander said that the hospital had been mis-identified as a target.
