Man charged over boy’s death during school activity at Safra Yishun
SINGAPORE — A man was charged on Thursday (15 September) in connection with the death of a 15-year-old student who died during an outdoor school activity last year.
Muhammad Nurul Hakim Mohamed Din, 22, faces one charge of causing the death of Puah Xin Yang Jethro at about 1.25pm on 4 February 2021, at the SAFRA Adventure Sports Centre in Yishun, according to a report by CNA.
He is accused of having committed a rash act not amounting to culpable homicide by failing to ensure that two leg straps of Jethro's safety harness were buckled and adjusted properly before the boy embarked on the rope course.
The student from Anglo-Chinese School (Independent) consequently died from multi-organ failure resulting from compression of his neck, and traumatic asphyxia. He had fallen off the rope course and was suspended in the loose harness with his leg straps unbuckled, according to the CNA report.
The incident happened during a high-element activity arranged by a provider of outdoor adventure learning services. Such activities involve participants who wear safety helmets and other safety equipment while clearing obstacles at a height.
All schools suspended outdoor activities involving high elements with immediate effect following the incident pending investigations, the Ministry of Education and ACS (I) said in a joint statement then.
The Outdoor Learning & Adventure Education Association said on the day of the fatal incident that it would tighten guidelines for its 20 members. It reminded all outdoor education providers to ensure that all processes are in place, including on the skill competency of instructors and best practices at various campsites.
